I support your doubts a 100%. I come from Quito, Ecuador and one of the foods you find on the streets are "chochos" very rich in protein (lupus sp.) sold with "tostado" a very good source of carbohydrates (fried corn). This is just one example. I'm sure there plenty more all over the world. The main problem though is the lack of hygiene in the preparation of some foods (due to lack of potable water, etc.) not their nutritional value.
Therefore, trying to "fortify" some foods seems a waste of money, time and energy which could very well be used on other more pressing objectives (e.g. hygiene).
