I know what you're talking about-- in JPEGView, there is a menu function called "Show Comments" and it will show comments if any were associated with an image. JPEGView doesn't let you create comments, however.

I was able to associate comments with GIF files, using GIFConverter, but when I saved the files as JPEGs, the comments were lost. I'm still searching for a program to do this.
