Stuart, Since you've asked several questions, I'll just refer you to a style guide that I've found helpful. If you don't find it readily available, I could photocopy the pertinent parts (1 or 2 pages) and fax them to you.I have not seen a cite that mentions HTML contributors nor have I seen cites with image, platform, etc., information.
