I am not sure about your first 2 requests but as I am responsible for OCRing camera ready display recruitment ads which can have extremely creative uses of different fonts and often have graphics and tables with different fonts I swear by Cuneiform over the more exspensive TextBridge and OmniPage Pro. Recognition is far better and you can ignore the myriad abreviations and acronyms that "appear" to be OCR mistakes but are not. I save in RTF format and it preserves italics and font face and size.
