Subject:      Regions in images

Do any of you know of image database servers/engines that support out of
the box labeled layers of image regions? We have a ton of medically-related
images with a lot of structures on them that students want to click on or
want to see highlighted.  We've built a Macintosh application that can
display such a beast, and I know we can build our own relational or
object-oriented database, but I was hoping not to.

As an aside, how important have you found this structure-clicking function to be in your different domains? From the drift of conversation that I follow, it looks like the focus is on images as a whole, and not on the parts.
