We have gotten some info pass forms at our circulation department that are different than the three part forms we have been using. The information is the same, but the patron comes with just one copy. It could conceivably be done on a word processor by the individual.

Have these forms changed? I feel the two shown to me by circulation staff were probably legitimate, but we here at University of Missouri-St. Louis did not know of the form change. In fact we recently ordered new forms and got the forms we all have been using.
