>Sandy,
We have also received the same forms from people sent to us from other
libraries. What is the deal? Did the forms and the process change? We
preferred the patron coming in with two parts of the form. We had a copy
for our records and one to return to the sending library.
We have gotten some info pass forms at our circulation department that are
>different than the three part forms we have been using. The information is
>the same, but the patron comes with just one copy. It could conceivably be
>done on a word processor by the individual.

>Have these forms changed? I feel the two shown to me by circulation staff
>were probably legitimate, but we here at University of Missouri-St. Louis
>did not know of the form change. In fact we recently ordered new forms and
>got the forms we all have been using.

>This is merely a question. Sandy Snell
